Overview
As a member of a multidisciplinary team, including health professionals and service providers from other sectors, you will develop and implement strategies for Transition Care Package clients. These strategies will aim to reduce the length of stay for patients admitted to Angaston hospital, and increase the use of health services by vulnerable groups. You will also strive to increase the likelihood of successful discharge of clients from hospital through supported rehabilitation in the community. Planning, implementing and evaluating a comprehensive and integrated range of services for eligible clients, you will coordinate the provision of services under the Transitional Care program within the Angaston locales. Working within an established program with countrywide networks and a good community uptake of the program, you will have access to a supportive network in a central regional location. For appointment at the AHP2 level, you will hold a recognised qualification in a relevant Allied Health profession, and be eligible for practicing membership of the relevant Professional Association. For appointment at the RN2 level, you will be registered by the Nursing and Midwifery Board of Australia and hold a current practicing certificate. With proven experience in the rehabilitation or re-ablement field, you will be proficient in prioritisation, meeting deadlines, and performing data entry, whilst working under minimal supervision. Past experience in the leadership, supervision and direction of less experienced clinical staff will hold you in good stead for this role.
