Position summary

Community Paramedicine (CP) is an emerging model of care where paramedics use an
expanded skill set to collaboratively support the chronic disease and primary healthcare
system in under serviced rural areas.

It is a client centred holistic approach focused on improved health equity through meaningful
and collaborative partnerships between the community member, GCH and GCH community
partners.

Community Paramedics Clinics (CP@Clinic) is not a centre-based program. Regularly
scheduled clinics are held in communities to enable easy access for disadvantaged
community members.

The purpose of this program is to combine a social health focus with primary health care to
overcome the specific social determinants of health (SDoH) impacting individuals and their
families’ health outcomes.

This position will work with community members within a Relationship Based Framework to
improve health outcomes, addressing health inequities with a particular focus on SDoH.
Using data, community voice, community leaders, and current research, a SDoH primary
care model will be implemented within community settings.

Key responsibilities

1. Lead the establishment of the CP@ program in line with the guidelines including
policies, procedures, and protocols to support the implementation and evaluation of a
new service and model of care.

2. Facilitate and/or provide health care in the community setting in the CP@ model
including regularly scheduled health clinics and ad hoc in the event of community
needs.

3. Increase the health literacy and access to at risk individuals and communities.

4. Work collaboratively with a variety of:

Stakeholders (internal and external) to maximise community address to health care

and improved health outcomes.

Service providers, including aboriginal cooperatives for clients to access the

appropriate support and health care.

5. Understand the local community and the impacts of SDoH specific to them.

6. Ensure the program is delivered within GCH’s policies, quality management and safety
systems and in accordance with CP@ scope of practice and standards applicable to
the program.

7. Lead the promotion of the CP@ program to the community with a focus on high-risk
communities and individuals.

8. Record, analyse and utilise data to inform the care provided to clients to optimise
outcomes for the client and community.

9. Complete evaluations in conjunction with identified evaluation partners and
collaborators.

10. Complete all internal and external reporting requirements on time.

11. Other duties commensurate with current skills and experience as agreed with the
General Manager or Chief Executive Officer.

Key selection criteria

Qualifications

Paramedic qualification.

Mandatory

Eligible for paramedicine registration.

Understanding of the Social Determinants of Health and the impact within the realm of

health equity.

Ability to think innovatively and flexibility to implement a new service.

Desirable

Experience in establishing a new service.

High level verbal and written communication skills that enable effective and appropriate

communication with a broad range of people at all levels.

Demonstrated skills and experience

Ability to maintain confidentiality at all times.

Demonstrated knowledge and application of computer software, including Microsoft

Office and the Internet.

Demonstrated ability to contribute to positive workplace culture and practices.

Licences and registrations

Current Victorian driver licence

Current National police check (less than 3 months old)

Current Employee Working with Children Check

NDIS Worker Screening Check
