Are you passionate about making a difference in the lives of vulnerable families? We are currently seeking a passionate Health Mothers Healthy Babies (HMHB) Outreach Worker to join our Social Work and Counselling team.
Join South West Healthcare's Healthy Mothers, Healthy Babies (HMHB) program where you will support pregnant women experiencing psychosocial complexity to improve the health and wellbeing outcomes for themselves and their infants. Women who access the program may have complex health and social needs requiring a range of service and coordination.
The overarching aim of the Program is to improve the health and wellbeing of mothers and babies, as well as reduce health inequity by addressing maternal risk behaviours and providing support during pregnancy. Working as part of a multidisciplinary team, you'll be responsible for:
- Providing assistance to women to improve access to antenatal, postnatal and other health and human services;
- Performing psychosocial and risk assessments, care coordination, navigation, and referrals;
- Providing psychological support by developing or connecting women to peer based group activities;
- Supporting eligible women throughout their pregnancy;
- Delivering key health promotion messages to support healthy behaviours in pregnancy and beyond;
- Providing assertive outreach services to the Warrnambool Region including across a range of settings such as client homes, outpatient clinics and within a hospital setting
- Providing health education about the impacts of smoking, alcohol and drug use, nutrition and breast feeding.
Position Overview
- Permanent part time, 48 hours per fortnight
- Warrnambool Campus
- Classification: Allied Health Professional Grade 2 / Community Health Nurse (based on qualification)
- Salary Range: $88,467 - $107,390 pro rata, plus superannuation
What You Bring
We are looking for someone who is dedicated to supporting vulnerable families. You will have:
- Tertiary qualifications in social work, midwifery, community nursing.
- Community based outreach experience
- Experience working with women, children and infants is essential
- Strong interpersonal skills to effectively engage with clients and stakeholders.
- Demonstrated ability to work independently and as part of a multi-disciplinary team including a capacity for reflective practice and a willingness to participate in clinical supervision.
- Demonstrated skills in engagement with diverse and vulnerable families
- Current Victorian Driver's Licence
