WHY THIS ROLE MATTERS
To provide bridging and brief intervention support to clients who are awaiting assessment and treatment commencement
To provide liaison, support and a range of interventions including case management to assist in improving the health & wellbeing for individuals, family members and significant others experiencing alcohol and other drug related harms to clients referred to Alcohol and other Drug Services, Substance Treatment and Recovery (STAR).
ABOUT THE ROLE
We are seeking a dedicated values-driven AOD Practitioner. You will provide an entry point to Alcohol & Other Drugs (AOD) services for people with AOD issues who reside in the inner north west region , through assessing client need, identifying treatment options and creating holistic treatment pathways.
This is a permanent part-time (22.8 hours/week) based in Coburg North, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
Responsibilities include
- Deliver up-to-date service and referral pathway information to all service providers
- Liaise with allied health and other health care professionals to obtain necessary medical and healthcare records
- Deliver evidence-based interventions in line with client-centred treatment that aligns with AOD training and qualifications
- Develop individual treatment plans in collaboration with clients including a range of interventions
- Implement individual treatment plans with active client participation
- Assist clients with building networks and post treatment support plans
- Provide assistance with the service delivery of other STAR Programs
ABOUT YOU
- Relevant tertiary qualification in Social Work or related discipline
- Minimum qualification Diploma Community Services
- Relevant experience in Social Service environment
- AOD Skillset or willingness to undertake within first 12 months of commencing employment
- A national police record check
- A current Victoria Drivers licence
- A valid working with children’s check
