About the role
Permanent part time opportunity to provide advanced clinical knowledge and consultancy social work services across Caboolture and Kilcoy Hospitals regarding individuals with disabilities, including consumers who are current and/or prospective NDIS participants, as well as patients with complex psychosocial needs and patients on the residential aged care pathway.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
In accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW Professional Practice Standards, deliver specialised social work clinical and consultancy service including functioning as a resource expert with advanced level clinical knowledge for patients with physical, neurological, psychiatric, developmental cognitive and sensory disabilities.
Work collaboratively with and provide high-level clinical advice and direction to social work practitioners and a range of health professionals, supervisors, relevant service managers and other stakeholders regarding social work and interprofessional management of NDIS participants and service delivery including prioritisation and changes in clinical practice, equipment and technological advances.
Provide professional leadership in the development of clinical practices, procedures and protocols that support the equitable access and health care of individuals with disabilities, engaging Hospital and Health Service and state-wide cross-discipline referencing where relevant.
