The Credentialled Diabetes Educator will be a valued member of the Albury Wodonga Community Health Diabetes Service. The purpose of the role is to provide diabetes education, assessment and intervention for patients and their families within the primary and community health service of AWH. To provide expert level consultation to, and integrated care planning with, internal and external service providers. To also participate in diabetes education service planning and development for continual service improvement that is aligned with local population health needs and service context. We are actively seeking candidates who meet the following requirements to apply for this exceptional opportunity.
- Relevant undergraduate qualification in related discipline with Postgraduate qualification in Diabetes Education/Management
- Member of the Australian Diabetes education Association
- Credentialled Diabetes Educator
- Current valid Driver's Licence
- Advanced current knowledge and skills in the clinical management of people with diabetes
- Knowledge and skills in contemporary service models and self-management approaches to chronic illness care
- Ability to plan, implement and evaluate diabetes education and management programs
- Excellent interpersonal, communication (written and verbal) and presentation skills, with ability to work as part of an interdisciplinary team.
Employment Type: This is a fixed tern part time (16 hours per week) opportunity for a period of 12 months.
